Who We Are

Assurance Support Services Agency started on January 12, 2016, and was founded by Gormah & Henry Gwagee. Our agency was established on the goal of giving our patients the opportunity to live a successful, independent, and dignified life in a community-based setting. We facilitate our patients’ physical, social, emotional, and financial needs to ensure a successful reintegration back into their own home and community.

Mission Statement

Assurance Support Services Agency strives to assist persons with disabilities in reaching the highest level of independence possible while on life’s journey. We also aim to make meaningful and positive health outcomes in the lives of the patients we serve by providing skilled support with the in-home care services we offer.

To reach these goals, we provide customized community services to assist our clients in meeting their specific health requirements. We also follow a patient-centered, multidisciplinary approach to rendering our services to ensure our patients achieve maximum state of function and independence in the comforts of their home.

What Is Colorado Choice Transitions (CCT)?

caregiver and elderly people smilingColorado Choice Transitions is a program designed to assist willing and able long-term care facility residents who receive Medicaid benefits transition to the community.

CCT is Colorado’s Money Follows the Person Program is a grant-funded national initiative to support individuals residing in nursing homes and other long-term care (LTC) facilities who are interested in returning to their own home in the community make a successful transition.

CCT services help clients to become more independent and, as a result, incur a far lower cost to the state for clients to live in the community compared to LTC facilities. Residents of LTC facilities can access community living options through the Options Counseling process. They may be referred to Options Counseling by family/friends, community members, providers, LTC social services, and/or through the Minimum Data Set (MDS) Assessment administered quarterly within LTC facilities. Services are provided during the transition process and the first year of community living, allowing clients to better adjust to their new living situation.
